Medicaid Service Coordinator

Yachad, the National Jewish Council for Disabilities is seeking a Medicaid Service Coordinator to assist individuals with Developmental Disabilities and their families in seeking out appropriate and meaningful services and navigating the realm of Medicaid-funded services. Responsibilities include:

  • Working directly with individuals and their families help identify individual’s goals and desired outcomes using person-centered planning
  • Developing Individualized Service Plans in conjunction with individual’s families and other service providers to outline individuals goals
  • Ensuring that services received are in line with goals outlined on ISP
  • Writing monthly service notes for each individual outlining all contact with the individual and any services rendered
  • Assisting families in all matters related to Medicaid and the Office for People with Developmental Disabilities (OPWDD)
  • Completing and filing service documentation in a timely fashion
  • Ensuring that internal records are up-to-date and meet state audit standards
  • Advocating for the needs and rights of the individuals you serve
  • Representing the mission and values of Yachad among families you serve as well as within the greater community

Qualifications

  • Strong organizational and interpersonal skills
  • Familiarity with Medicaid Waiver services strongly preferred
  • Experience working with individuals with disabilities preferred
  • Ability to travel to locations throughout the five boroughs to visit clients
  • Strong writing skills
  • The desire to improve the lives of individuals with disabilities

Position is currently part-time with the possibility of becoming full time.

Yachad,  a division of the Orthodox Union, works toward the mission of promoting the inclusion of individuals with disabilities into mainstream Jewish life.  Our summer department fulfills this mission by partnering with mainstream camps across the US and Canada.

Openings include:

B. Assistant Program Directors (males only)

C. Counselors/Job Coaches

 The opening are in various locations in NJ, LA and PA. Prior experience in working in a special needs setting is only required for supervisory positions.

Administrative Assistant, Yachad

Yachad, The National Jewish Council on Disabilities, is seeking a motivated and detail-oriented individual for an administrative position. Responsibilities include:

  • Assisting in the day-to-day functioning of a nonprofit organization
  • General administrative projects
  • Assisting with organization billing functions and managing timely payment of bills
  • Assisting families of developmentally disabled individuals in acquiring funding from various state-run programs
  • Following up with funding agencies on the application progress of our individuals
  • Fostering relationships between Yachad and potential new families
  • Tremendous room for growth within a growing organization

Qualifications:

  • Excellent interpersonal skills
  • Excellent organizational skills
  • Some bookkeeping experience preferred
  • A desire to improve the lives of individuals with special needs
  • Familiarity with Medicaid Waiver and ACCES/VESID services helpful
  • Some clerical experience preferred

Position is currently Part time but could lead to Full Time.

Creative Design Coordinator

Position Summary: Staff member who is responsible for designing product lines for Yachad Gifts, in which gift baskets for a host of different occasions are designed, packaged, and sold by individuals with disabilities. Responsibilities include but are not limited to designing and creating gift baskets, ordering new products, and posting new products along with their descriptions on Yachadgifts.com

 Essential Functions:

  •         Designing and implementing Yachad Gifts product lines
  •         Responsible for placing products on Yachadgifts.com
  •         Writing product descriptions
  •         Ordering gift basket supplies
  •         Producing baskets

Job Qualifications:

  •       Must be highly organized and able to work under a deadline
  •       Excellent time management, organizational, and multi-tasking skills
  •        Strong writing skills
  •        Must be familiar with Microsoft word
  •        Previous experience in the gift basket business a plus but, not required
